Failure Reporting

Equipment failure reporting consists of data collection and data analysis steps:

  • Equipment Malfunction Reports are used to collect failure data from maintenance personnel
  • Data analysis includes data merging and assessment steps
    • Merging is the process of choosing a select dataset of failures events based on common parameters defined by the analyst
    • Assessment is generating failure statistics for the select dataset
  • Equipment taxonomy and master data provide the infrastructure for failure data collection, merging, and assessment

Failure Data Collection

ISPM Malfunction Report

  • Initiated for the following reasons:
    • To notify Maintenance of required repairs
    • To record repair details and failure consequences
    • To document failure discovered during preventive maintenance and inspections (regardless of the need for follow-on work)
  • Failure coding is used to standardize data collection
    • Failure codes are equipment class specific, e.g. for a pump you see only pump failure codes
  • The failure event dataset is compliant with international standard ISO 14224:2006
    • System data validations ensure proper and complete data are collected in each of the two processing stages (required data are different in each stage)
  • Two processing stages
    • Problem Report: Used to report an equipment malfunction and/or initiate repair work
    • Repair Report: Used to report repair details and event consequences once repair work is completed
